What is the navigation program?
The nurse navigator assists patients and their families from the time cancer is diagnosed through treatment and follow-up care. We provide critical support to guide patients through each step of their care and make the journey as stress-free as possible.
Benefits of nurse navigation
- A skilled and caring expert in navigating your care
- Care that is streamlined and coordinated
- Personalized treatment plans
- Help with coordinating with oncology specialists, surgeons and support team members
We make sure all patients with cancer have the right people on their team at every step of their journey from diagnosis through recovery and survivorship. We can help:
- Streamline care and communication for physicians and patients
- Reduce delays in treatment by coordinating access to the right care at the right time
- Educate patients about medications, symptoms and treatments
- Link patients with needed resources such as support groups, financial assistance, and transportation
- Guide patients through an often complex healthcare system.
